Diagnosis and surgery of tumors of parapharyngeal space

Parapharyngeal space (PPS) is deep neck space extending from the base of the skull to the hyoid. It may be divided into pre- and poststyloid compartments. Tumors of PPS comprise less than 1% of head and neck neoplasms. Wide variety of histological types have been described in this location. Imaging methods determine extent of tumor, assess its resectability, relationship to carotid artery and skull base and intracranial spread. They are critical for differential diagnosis, further workup and choice of surgical approach. Fine-needle aspiration biopsy contributes to preoperative evaluation.
Majority of PPS tumors are benign, and surgical excision is the primary treatment. Surgical approaches can be classified as: transoral, transcervical-submandibular, transcervical, transparotid, modified transcervical (transcervical-transparotid, transcervical-transmastoid, transcervical with mandibular osteotomy), transmandibular and lateral skull base approaches. Anatomy, pathology, diagnostic methods, indications and tactics of different approaches and their complications will be presented both on the review of the current literature and personal experience of senior presenting author with treatment of 112 tumors of PPS during the last 10 years.
